I’ve used a door like this in an old folks home, it DOES lock but when the latching mechanism gets pushed in when the door is closed it unlocks it, you have to have the door already shut then it will stay locked, basically a way of making sure no one locks everyone out of the bathroom, door can’t be locked unless someone is actually inside

Depending on the setting, it may have been intentional. US medical facilities need a way to override the lock and simply not putting it in would be an easy solution.
