I used to have to talk to UHC doctors trying to pull people early before they were finished treatment from our hospital. Their docs- often immigrants- would try to fool/lie to me- utterly shocking actually. "They deny claims in tranches of 50 or 100 at a time! Large insurers like UnitedHealthcare (UHC) often face scrutiny for high rates of denials. Automated, batch-style denials are a point of contention and class-action litigation, typically result from billing triggers or algorithmic reviews."

Please note, anyone using UHC as a health insurance company needs to understand that they are already dead.
Everyone, I want you to look up your health insurance provider and check if they are For Profit or Not-For-Profit... if they are For Profit, they are financially motivated to let you die.
Mine is a NFP-Mutual (premiums are based on amount paid out, not a profit target)... in 21 years I have never had a claim denied.

So, my insurance company is BCBS-IL (please note that some Blue Cross/Blue Shields are For Profit organizations, so check)
The key component is a complete lack of shareholders.... basically it is owned by its members (like any Mutual Insurance Co.)... and any profits earned are held as a reserve to ensure financial stability not pay out dividends or share.
