Vaccines aren't 100% effective individually. They're quite effective, maybe something like 90-97% depending on the vaccine, but you can still get sick even vaccinated. The real strength in vaccines is herd immunity, but that requires EVERYONE vaccinated. Otherwise, anti-vaxxers are disease vectors. (Plus there's also immunocompromised people, infants that haven't been vaccinated yet, etc.)
Climate change deniers are also a big risk but on a longer term, and most can't directly cause harm.

They're not directly causing harm, but they are restraining us from stopping the people who are. We could stop a lot of pollution very quickly if not for massive pressure both from "grass roots" denier movements and fossil fuel lobbyists. They might not hurt us directly, but they're content to make us watch - Clockwork Orange style - as the world burns.
As for herd immunity, yeah, that's why I put "generally". Antivax is a mass death distaster, climate change is the apocalypse.
