That phrase is doing some absolutely Herculean fucking lifting.
Safeguards introduced after catastrophic harm aren’t exactly safeguards — they’re lessons written in somebody else’s suffering. “We’re deeply sorry” may be sincere, but patients deserve systems designed to catch these mistakes before an apology becomes necessary.
