Cubone
Almost everybody knows about the sad tale of Cubone. It wears the skull of its mother on its head, never allowing its face to be shown. During the full moon, Cubone is reminded of its mother's face, and weeps. In fact, little orphan Cubone cries so much, it's responsible for the stains on the skull.
Grimer
Grimer is literally a sentient, moving pile of filth. It feeds on the waste pumped out by factories. The problem is, Grimer is walking pestilance. According to the Pokédex, "wherever Grimer has passed [...] no plants will ever grow again". And when two Grimers love each other very much and- joking. Whenever ANY two Grimers join forms, the fusion spontaneously creates new and frightening toxins, which is not at all terrifying.
Muk
Muk is Grimer's evolution, and of course is about 500x as bad and nasty. Muk smells so bad, it makes people faint. The rudimentary sense of smell it had as a Grimer is burnt away from the toxins that make up its body after evolution. Literally all of Muk is toxic, and touching one sends humans to bed with a horrible fever. Plants, trees, and particularly small Pokémon die instantly upon contact with a Muk and the, ahem, "juice" it oozes constantly. Even a single drop of Muk's effluence will stagnate and poison an entire swimming pool. And 10 year olds just casually carry them around in a ball.
Glalie
Glalie is another one of those Pokémon, like Duskull and Gourgeist, that makes you understand why people keep and train Pokémon: to protect against actively sadistic dudes like them. Glalie's power over ice is so powerful, it can freeze its foes and/or prey completely solid. And then "After immobilizing its foe in ice, this Pokémon enjoys eating it in leisurely fashion."
So you get to stand there, frozen in ice, while this guy chomps casually on you, uncaring and unsympathetic to your pain.
Gothita, Gothorita, and Gothitelle
I included Gothita and its two evolutionary forms as one entry, because their creepy Pokédex entry expands in context across the three forms. The entry for Gothita explains that it stares at humans and trainers alike, fervently, all the time, as though Gothita is seeing something you're not. Creepy enough to have a little pink goth baby staring at you 24/7, but oh, it gets even better.
Gothorita further explains just how powerful of a Pokémon it is, and just how creepy. See, Gothorita gets bored, and just wants friends. To do so, Gothorita stands outside of sleeping children's windows and uses its hypnotic psychic abilities to telepathically control them like puppets, and telekinetically move them around, ALSO like puppets! It then flies around all night with them, through the sky, pretending the children are actually interacting with it. Then, for funsies, it will track down travelers and get them dangerously lost, so now it's both creepy AND actively a jerk.
Gothitelle just ramps it up from there. By now, its psychic powers are so strong that it creates visible-to-the-eye distortions in the night sky, bouncing, twisting, and reflecting the starlight into crazy shapes, and that's not even while it's actively using any psychic abilities. It begins staring at the stars just as often and as weird as it stares at trainers and humans, because (of course) the stars are telling it the future. And for the cherry on the creepy goth sundae, it will one day turn to look at its trainer and will simply start sobbing. Why? Oh, because the stars told it exactly when you, the trainer, will die, and it can't possibly express that information to you. It just cries for you instead.
Jellicent
The royal octopus of the Pokémon oceans, Jellicent is described in the Pokédex as having a body "made of mostly seawater". How, you may ask, can a stupid sack of salt water be creepy? Because it's the Flying Dutchman of Pokémon. It is known to sailors in the Pokémon world that you do not, under any circumstances, try to sail through Jellicent-infested waters. The fates of the ships and crew is stated as "All sunken, all lost, all vanished". It's even said that Jellicents have built themselves a castle on the sea floor, out of all of the shipwreck debris and bones of the ships and crew they have sent down to the briny deeps.
Shedinja
Another well-known creepy Pokémon, Shedinja is the shell left over from Nincada's evolution, wherein it molts. Shedinja doesn't move, at all. In fact, there is almost zero indication that Shedinja is anything more than a molted exoskeleton, until it starts attacking you. Worse is the tale that states looking upon the empty hole in the back of a Shedinja will rip your soul from your body and trap it inside.
Guess what is staring you right in the face if you're a trainer who throws out their Shedinja to use in battle? Oh yeah, that big huge gaping soul-sucking hole.
Cofagrigus
Cofagrigus is the evolution of Yamask, and it is said that it is created by Yamask forcing a grave robber to wear its mask. Cofagrigus looks like any old Egyptian sarcophagus, with variations in color and design as you would expect. The one thing that's unexpected is that when anyone gets near enough a Cofagrigus, it flings itself open, snatches you with 4 shadowy arms, yanks you inside, and slams itself shut. Archaeologists, grave robbers, a 10 year old kid on their Pokémon trainer adventure, doesn't matter. It will eat you, and then turn you into a mummy, trapping you inside itself for eternity.
Mismagius
The evolved form of Misdreavus caused by giving it a Dusk Stone, Mismagius escalates Misdreavus' ghostly mischief into straight, literal nightmare fuel. While Misdreavus enjoys simple pranks, like hair pulling, biting, or making creepy noises, Mismagius simply tries to use every and all dark creepy magic ability it has. Simply hearing a Mismagius' cry is enough to induce severe migraines in anyone listening. The Pokédex states that "its cries sound like incantations", which is probably because that's just what they are. The incantations are used to curse with misery, or in very very rare cases, bless with joy. As well, the incantations of Mismagius can cause anyone listening to fall into a deep sleep, trapping them in a terrifying dream world until the Mismagius is disrupted.

Jellicent is a jellyfish, hence the mostly water part. ISailing through masses of floating jellyfish (like Man-o-wars) was difficult 1/2
