If you look close there are little cleat-like spike/knobs moving when he turns the crank. They appear to be turning the cob as it moves down, but when the cob gets low enough, there is nothing holding it vertical anymore, so the spikes pull it out instead.
As the wheel’s pins move to the left, it twists the corn against other pins on the inside of the cylinder, knocking off the kernels.
When the cob gets to the bottom, it’s caught by the pins moving to the right at the bottom of the wheel, and that drags the cob up and out of the device.

The diameter of the lower part is just big enough for a cob, so it’s pushed against the pins.
No, it’s not perfect, but it’s good enough to save most of the “picking out corn cobs” work.
