Unfortunately they changed the style of coupler used, so a pic of the new one wouldn't really help you out.

I think it started off as just a circles and the bolts wore into the edge more and more until it almost sheared through.

Yes. There were rubber bushings in place to absorb some impact from the bolts turning. Then they fell out...
