Pretty much every engineering disaster ever (eg: fukushima, chernobyl), is because of humans cutting corners and not listening to engineers.
Tech is usually perfectly safe when you follow the guidelines and physics, what's not safe is when you let executives/money corrupt things.

Usually engineers themselves are at odds with the human in them too, "but the math says vs i'm lazy/boss is mad/could get laid off/etc"
