I think the disconnect comes from not seeing the boy in the two panels prior, the reader may mislead themselves into believing it was the same child. I know that's what I did my first time through.

Patreon is a platform used by various types of content creators to earn subscription money from fans. Some artists will make their work available to everyone for free eventually, but offer early access to subscribers. In this case, the artist is effectively asking for donations so they can afford to keep creating enjoyable content, and the early access is just a nice perk. It's not a normal business model because it requires a certain amount of generosity from both the creators and consumers.

The boy said something naughty earlier and the incriminating speech balloon didn't go away. On the final page, we see his sister ("child of the month", according to one of the framed pictures) had also said something incriminating at some point in the day, and her speech balloon never went away either. And it reveals she was involved in a murder.
