The DM is never at disadvantage, they can lie about roll results as needed to punish or reward players as the campaign requires.

A DM should never fudge their rolls! I think the knowledge that your DM doesn't is important for the sense of danger and consequence... As well as just sometimes you get unlucky!

Depends on a lot of things. Properly balancing the difficulty of an encounter can be really challenging, and a tpk is deceptively easy to accidentally create. There should be risk, there should be few fibbed rolls, but at the end of the day the point is to have fun, and it's the dms job to make that happen.

All the best DMs cheat on dice rolls just a bit, to the advantage of their players. Remember that the goal is to have fun, an encounter where just one team member barely survives is a lot more fun than a total wipe
