I can't speak for developing/"third world" countries, but normally yes they should be strapped/chained down. Anything hauled on a truck should be secured especially cars.

In Europe the norm is that each wheel is strapped down plus the car that is over the cab has also a safety chain to prevent exactly this from happening.

In Canadaland, they're always strapped/chained. Worked at a Toyota dealer for almost 17 years and always saw them properly secured. Sadly... I am now once again working as a mechanic, but in a Hyundai dealer closer to home, vehicles coming in are secured as well and it's not the same truck company either. While the accident wasn't the driver's fault, totaling two new cars will be worth a paddling.

In the US and probably most other countries as well. Fairly easy to assume the cars that fell off probably were not properly secured and that's not the van driver's fault, but the ACCIDENT absolutely is. Him cutting off the truck and hitting the other car is 100% the right white van's fault and I suspect the truck's insurance will argue that none of the rest would've happened if the van hadn't hit the truck in the first place. How much of the blame is probably up to a judge/jury.

truth be told what probably happened is the second car was likely the one that wasnt strapped down and when the trucker slammed on his brakes the second car came foward, hit the first car and they both went over.

Generally speakihng, you strap down for standard road pressures. Being struck by and then have sudden deceleration from the drag of a vehicle being pushed by yours is not standard conditions. It is somethihng you can't plan for. The vehicle would be liable fro what I see.

where i live you need a minimum of 4 straps... one for each corner. and each strap must be able to hold 50% of the vehicles weight ( inside its Working Load Limit... not the breaking load limit of the strap ) and anything over a certain length ( i think 8 or 10 feet ) you need to add 2 more straps. I've literally had an inspector say to me it will not pass inspection if the load can not be dangled upside down from the over turned truck.

actually i think that 50% i mentioned is too low. i'll have to pull out my book again. but for our company specifically to play it safe and pass inspection if we are hauling a 10,000 load than each strap's working load limit will also be rated for 10k lbs. we usually do 2 chains each with a split bridal and 4 straps at each corner. add more for anything longer than 8 or 10 feet.

Im not sure, the insurance will say the driver did not secure his load properlly and probably only cover the damage to the transport truck and not the new car that got smashed.

Yeah, same as if the trunk had dumped over. Emergency braking (although you'd hope straps would account for that) is an unusual circumstance caused by the idiot. Opposite happened to me once, the semi clipped my back left, and I was left facing the front of the semi in my driver side window - was freaky.

nah, at least partial fault to the driver of the truck because of unsecured load, the two cars were preventable damage had they been properly secured...aside from the fine for the driver that will be split 50-50 or something like that...

depends on legislature, but normally everything is the responsibility of the driver, securing the load, ensuring the safety mechanisms actually work, etc...would be difficult to redirect the blame to some inspector or the manufacturer of the device...normally the driver should have noticed the mechanism being about to fail in some way, thats part of his responsibility...but i also dont see any straps or chains or anything attached to the cars, if something broke there would be parts left...
