There's two forms of empathy: Affective empathy, i.e. empathy for those literally right in front of you, and cognitive empathy, i.e. empathy for people you don't directly know. Lots of people have affective empathy, which is why they can care about family members, friends, neighbors, etc. However, cognitive empathy is either underdeveloped in many, or it can be short-circuited by "othering" certain groups of people. Hell, even affective empathy can be cut off by othering.

I'd say it's sort of both. When I started learning the darker sides of US history & using the experience of others in political conversation with my right-wing family, they were quick to ridicule me & my "liberal education." So while most right-wingers (like my family) seem incapable of empathy toward other humans they don't personally know, I'd argue that others aren't allowed to express that without fear of ridicule & being ostracized by their community, which can be a powerful thing. 1/2

Apparently my grandfather and his brother were the holdouts who didn't want to sell the land but another family member took the offer and fucked off to North Carolina with the money. That branch of the family I don't even know got rich. Houston got to build Intercontinental Airport (now named after Bush) and my Grandfather and his other brother got nothing. Remnants of their house were still at the end of a runway until about a decade ago.
