This is how the modem fits into the ISA riser card that it plugs into while inside the Acer ACROS 486SX/25.

The front of the modem contain what I believe to be the model number, FM-9648HR/1 KB3110-06.
The back of the modem lists the usual FCC rules along with a sticker that might contain the serial number.
The dip switches you see there are set exactly how they were set when I received the Acer ACROS 486SX/25. If you need the correct settings for whatever reason, those should work.

I work with a lot of equipment that still uses DIP switches. They are not fun.
