I believe the foot of the ladder that's on the left and closest to the camera is not on any surface, but hovering in the air, with the man on the ground attempting to provide support for it. When the man on the ladder wobbled, the spotter failed to provide enough support, and the whole contraption tipped over. Apologies if you were being sarcastic and I missed it, though.

Wouldn't be surprised if he has some shoulder damage. The way he lands on his right shoulder and its bent behind his body. I mean, its not really an issue if you stretch your arm like that yourself, but when it because you fall on it, could dislocate your shoulder or tear some ligaments.
