I was at a company where we had a product we only made once a year. In my 5 years there we had needed to redo 3 batches. I sat in on the production meeting, asking what extra precautions they were taking (I wasn’t in production, just a side department that had witnessed. They brushed me off, “I was overly concerned”.
This one took three attempts to make good product, at a net loss and I somehow got dragged into it on the last attempt.

And then getting one step of it slightly wrong, because you haven't touched it in for fucking ever and then an entire teamwide missive goes out and you get 3 separate email about the correct procedure.
