Let's assume that is his right hand. I don't know about you, but I've never taken a picture with my left arm while holding it crossed in front me. And my right elbow also pulled in front with that hand pointing to the right. But, if the person taking the photo were sitting next to him, then no bizarre contortions would be required. (unless you want to argue that his left hand is shaped like a normal right hand. i'll leave that one to Occam)
