I spent about 3 hours on the freeway today (I'm in CA) and repeatedly saw motorcycles splitting lanes and passing cars in 60-70 mph traffic.
Definitely at speeds from 0-15mph. Helps prevent riders from getting rear ended which is very common.

That said I play it a bit by ear, not too fast, not too slow. If I don't fit, I don't split. Nor do I zoom by at 65 with stopped traffic.

Sort of. Utah has some limited access roads that aren't freeways with limits of 50-55. They were likely trying to exclude those.
It's just filtering forward at stop lights, not lane splitting. pedestrians and cyclists should not be crossing in the middle of traffic.

I've done it at that kind of speed on a motorway (highway...? freeway...? idk really) when the traffic is going at 50mph or so.
How you do it is what makes it safe/unsafe. Bikers here tend to be hyper-vigilant. Of course we're all human and all make mistakes.
I've yet to meet a biker who didn't think they weren't hyper vigalent. I've also yet to meet a biker who admitted to dickhead driving.

So what happens if someone has an emergency and needs to open their car door? Or what if someone, who has right of way, changes lanes?
Then you look. It's pretty simple. As a biker you also need to watch out for that stuff and need to be able to stop in time.

You never have the right of way if you're changing lanes. Law imposes a duty on you to change lanes safely.
You DO have the right of way changing lanes if traffic is clear in the lane you're changing to and you have your signal on.
Then they should check their mirror first to make sure they can safely open their door. Kind of obvious isn't it?
