Explanation: Flayed Ones are twisted and ghoulish Necron terrors afflicted by an ancient infection that breeds a hunger for organic flesh in them. Flayed Ones act as specialised close combat troops. They appear from an unknown pocket dimension of their hideous kind to join the Necron armies in battle, though never by invitation from the Necrons themselves.
These loathsome creatures were once Necrontyr who managed to retain some of their original consciousness when they were transferred into their living metallic bodies of necrodermis, but were cursed with a terrible disease, manifesting a hunger for flesh that cannot be satisfied and that eventually drove them to madness.
Fun Fact: In the northeast corner of the galaxy exists an old Necron kingdom known as the Bone Kingdom of Drazak, which is a haunt for Flayed Ones; every Necron save its ruler, Valgûl the Fallen Lord, is a Flayed One. Whenever the kingdom suffers from "food shortage", Valgûl issues a time of bounty, sending his people out in search for more.

Ah Necrons. My first 40K army, and one of my two favorites. I always got the feeling that if they could be, other Necrons would be creeped out by the Flayed Ones like normal people are creeped out by axe wielding clowns.

I prefer the old lore where Flayed Ones simply missed their living flesh so much they wore the flesh of their victms for comfort.

As of Twice-Dead King, it's a blend of both; An intense, uncontrollable urge to be living flesh again. Which means cloaking themselves in flesh, "eating", etc. Biotransference was supposed to remove the urges tied to living organisms. A rogue C'Tan poisoned the process and to a greater or lesser extent all Necrons have base instincts that prey on their mind. Flayed Ones are when the curse overwhelms their self-control.

It wasn't a rogue C'tan; it was Llandu'gor the Flayer cursing the Necrons as they were destroyed (instead of shattered) during Szarek's rebellion at the end of the War in Heaven.
