Depends on what kind of vision impairment you have, but the small hole filters out light coming from different angles which your eye has trouble focusing at the right spot on your retina. The light coming straight through doesn’t need to be refocused.

There's a minutephysics video about it. It does work but yeah, really reduces your field of view and the total light reaching your eye is less so it's dimmer. Cool but unreliable
